Psyllium (Metamucil), what is this drug?
FIber
What is the action of Psyllium?
adds bulk and size to fecal matter
what is psyllium used for?
treat consitpation and bowel irregularity?
what are the side effects of psyllium?
severe flatulence and abdominal bloating
what are the contraindication of psyllium?
prescence of undiagnosed abdominal pain, can cause organ to inflame, rupture, and spill over into GI cavity
what is a patient teaching with Psyllium?
take with a full glass of water
What is Bisacodyl (dulcolax) action?
irritate GI mucosa pulls water into bowel lumen, moves through bowel quickly not allowing feces to absorb water
What are the uses of Bisacodyl?
relief of constipation, bowel prep for surgery or medical examinations
What are adverse effects of Bisacodyl?
abdominal pain, cramping, N/V and weakness
what are the contraindications of Bisacodyl?
undiagnosed abdominal pain, intestinal obstruction or fecal impaction
What liquid can interact with BIsacodyl?
Milk, it can decrease absorption and enteric coating of drug causing early release
Bisacodyl can interact with what meds?
Antacids and H2 receptors
what do we use Magnesium Hydroxide (Milk a Mag) for?
rapid evacuation of stool, fecal impaction, and to clean bowel for endoscopic exam
Why is Milk a Mag not safe for prolonged use?
Can cause imbalance in electrolytes and fluid (dehydration)
What do patient with renal impairment experience when taking milk a mag
Hypermagnesia due to being absorbed systemically
What is the contraindications when giving Milk A Mag?
GI obstruction, gastric retention, colitis, and bowel perfusion
What is the use of lactulose?
Decrease hepatic encephalapothy
decrease ammonia levels
how do we know Lactulose is working?
Improved mental cognition
2-3 soft stools daily (diarrhea and loose bmβs do not indicate its working)
What are the 3 NOTβs of Lactulose
NOT going to decrease portal HTN
NOT renally excreted (pee)
NOT going to decrease ascites
What is the mechanism of action of Lactulose?
Pulls water into intestinal lumen
is a dissacharide that is not absorbed
Peclanotide (Trulance, β-otideβ) drug class?
GC-C antagonist
What is the mechanism of action of Peclanotide?
Increase secretion of chloride and water and binds to the GC-C of intestinal epithelium
What is the use of Peclanotide?
soften stools and stimulate BMβs
when do we take Peclanotide?
we take this 30 min before our first meAL
What is the black box warning of Peclanotide?
Not given to children <6yrs old due to leading death of dehydration
what are the adverse effects of Peclanotide?
abdominal discomfort and diarrhea
