What is the serum bilirubin test used to analyse
Liver metabolism and excretion
What does ALT measure?
Hepatocellular damage
What does ALP measure?
Used to assess biliary obstruction (cholestasis)
What are the 2 forms of bilirubin
- Unconjugated (indirect) = bound to albumin, not processed by liver
- Conjugated (direct) = water-soluble and excreted in bile
What is bilirubin?
Breakdown product of heme from RBCs
Metabolized by liver and excreted in bile
Unconjugated bilirubin formula
Total bilirubin - conjugated bilirubin
What does elevated bilirubin (hyperbilirubinaemia) levels indicate?
- Increased RBC haemolysis
- Liver damage/disease
- Cholestasis.
What causes increased unconjugated bilirubin ?
Haemolytic anaemia
What causes increased conjugated bilirubin?
- Cholestasis (gallstones, bile duct obstruction)
How to determine if increased bilirubin is caused by pre-hepatic or post-hepatic reasons
Fractionation of bilirubin
Increased unconjugated means pre-hepatic
Increased conjugated means post-hepatic
What does increased ALT indicate?

Causes of increased ALT
- hepatitis
- viral, alcoholic, autoimmune, drug-induced.

What does increased serum ALP concentration indicate
Cholestasis = obstruction of bile flow from liver
