Lab 3 - Advance Airways Flashcards
[Trach Suctioning Complications]
Hypoxia
S/s
Prevention
S/s: SpO2, cyanosis, cardiac dys, PVC, cardioresp arrest
P: limit pressure/duration, <1/2 diameter, manual vent, hyperoxygenation, no routine, limit catheter passes
[Trach Suctioning Complications]
Cardiac Dys/Arrest/Death
S/s
Prevention
S/s: increased HR from decreased arterial O2; decreased HR from vagus response
P: assess hypoxia, stop suctioning, o2 supp, manual vent
[Trach Suctioning Complications]
Trauma/Damage
S/s
Prevention
s/s: aspiration of blood and decreased air entry
P: lowest lvl of pressure, gentle, don’t go against resistance, withdraw before suction, lub suction, limit number of passes
[Trach Suctioning Complications]
Infection
S/s
Prevention
s/s: increased abn secretion, colonization, increased HR/RR/temp
P: sterile/aseptic, gentle, hydration/nutritional/metabolic needs, no routine, oral care, PPE
[Trach Suctioning Complications]
Hypotension/Hypertension
S/s
Prevention
s/s: change in BP
P: stop suction, supp O2/vent, calm, pain control
[Trach Suctioning Complications]
Atelectasis
S/s
Prevention
S/s: decrease air entry, change in chest x-ray
P: limit - pressure, short duration, increase vent, appropriate size
[Trach Suctioning Complications]
Vagal Stimulation
S/s
Prevention
s/s: cardiac dys, decrease HR
P: max O2 supp, be calm
[Trach Suctioning Complications]
Bronch- constrict/spasm; Cough
S/s
Prevention
s/s: change in air entry, wheeze, same as hypoxia
P: bronchodilators, vent with resp effort, calm, if unable to vent - sedate
[Trach Suctioning Complications]
Obstruction
S/s
Prevention
s/s: can’t ventilate, can’t suction
P: call for help stat, prepare to change artificial airway, attempt to vent til help arrives
[Trach Suctioning Complications]
Increase ICP
S/s
Prevention
s/s: correspond with increased BP and coughing
P: Aerosolized lidocaine 15 mins before suction
[Trach Suctioning Complications]
Distress/Pain
S/s
Prevention
s/s: visual signs, increased HR/BP
P: Explain, be calm, reassuring. Look for discomfort, SOB, role in coughing
