Intervention:
1. Request order for culture on needle
2. d/c infusion

- Displaced, puncture vein, leaking blood, leaking at site
S/s: cool, swell, discomfort, edema, no blood return, leaking at site
Intervention:
1. d/c infusion and remove IV
2. pulse and cap refill
3. apply warm compress (TID-QID)
4. Elevate limb
5. don’t massage

- Infiltration of irritant drug, causes long term damage
S/s: red, edema, hard, cool, burning, tissue slough, poor infusing
Intervention:
1. d/c infusion
2. notify HCP
3. policy
4. new site (opposite side)
5. elevate limb

A
- Inflammation, irritation by solution, trauma, too small/large vein
S/s: edema along vein, sore/hard/warm, redline, slowed infusion rate, tender
Intervention:
1. d/c infusion
2. warm compress
3. don’t rub

- obstructed by thrombosis l/t venous wall injury
S/s: infusion stops, pain/swell/red
Intervention:
1. Remove IV
2. Start new location
3. observe site

A
- Sudden adverse rxn to IV med (admin too quickly)
S/s: flushed face, h/a, tightness in chest, irreg HR, LOC, cardiac arrest
Prevention: flush @ prescribed rate, diluted med
Treat: stop infusion, notify, dex 5% in water @TKO rate
