Watch the horse go before and after use hoof testers
EXAM: when jog a horse?
Jog in hand before and after flexing a limb
Initial Ortho Exam
Triage!
- -distress: pain, blood loss
- -stability
Hx
Signalment --age, sex, breed --use prior to and during injury Lameness onset --swelling? Bleeding? --changes since onset? --last time shod? --Current exercise? --Current housing? Blood tests --HYPP --EPM --Vax
Overall Assessment
From a distance --how standing? Compare left to right --how are shoes worn? Conformation faults - note but keep in perspective Don't take eyes off horse!
Hands On Ortho Exam
Be consistent!!! Palp: jug, muscle, tendon, bone, jt Flex Hoof testers - be sure go all the way around the foot! Walk, jog, lunge, flex Rider up
Ortho Exam Dx
Lameness exam
Nerve blocks
Jt/sheath/bursa blocks
Synovial fluid analysis
Ortho Exam Imaging
US RADS (contrast) Bone Scan MRI CT
Where should your handler stand?
Same side as you!
Why care if the left jug fills?
Under it, RLN - close anatomic proximity. Make sure both veins distend before INJ
Palpate distal limb

Stand so that you’re facing the horse’s back end
Stifle palp
Find crook in back of leg - go straight forward –> tibial crest –> go up –> patellar ligament
Find middle first because it’s the biggest

Flexions
Let shoulders, back and core take any movement
Horse’s leg should be tucked under body
Can cause false soreness if flex wrong
Wear gloves if holding onto foot

Cruciate Tears
Don’t need to do anything to dx
Traumatic event
Joint trashed - start digging hole
Other flexions
Don’t forget to check for necks! Should be able to bring nose to shoulder
AAEP Lameness Scale 0
Lameness not perceptible
AAEP Lameness Scale 1
Lameness difficult to observe
Lameness exam in motion
Jog in hand --watch how turns, stops and starts --flexion tests, wedge tests --watch from front, back and sides Lunge U/S
AAEP Lameness Scale 2
Lameness apparent under certain circumstances
–weight carrying, circling, inclines, hard surfaces
AAEP Lameness Scale 3
Lameness consistently observable at the trot
AAEP Lameness scale 4
Lameness obvious at walk
Lameness scale 5
Minimal WB
Dx Tests
Blocks - perineural, jt, bursa
