Removal of fuses
Ensure all services have been isolated and all equipment is off/stopped.
1) Place finger firmly in centre of fuse holder
2) With free hand, unscrew fuse holder using a fuse key
3) When unscrewed, grasp fuse holder, turn head away, remove fuse holder
4) Remove fuse and keep safe, replace empty fuse holder to maintain the dead front of the fuse panel
5) Repeat for second and third fuse way
Replacement of fuses
1) Ensure you have right fuse, remove empty fuse
2) Test for voltage using approved test equipment across empty fuse way is dead. This proves the equipment isolating switch/breaker is still open. If zero volts is indicated replace the fuse, repeat for 2 and 3.
3) Ensure you have correctly rated fuse. If the wrong fuses are used cable burnout could occur.
4) Averting eyes, locate the fuse carrier smartly onto the bus bar.
5) Tighten with fuse key whilst holding the fuse carrier firmly in place.
Restoration of supply
1) Inform the senior rate the work is complete and obtain permission to restore supply.
2) Complete section 2 of LOTO safety certificate
3) Collect fuses/keys and label parts B and C
4) Remove part A of LOTO label from fuse holder
5) Test for dead using GPTME, if dead replace fuses
6) Return all LOTO labels to authorising senior rate
7) Complete safety certificate sections 2A
8) Close LOTO index entry
9) Completed LOTO safety certificate to be placed in part 6 of LOTO log
10) LOTO labels are destroyed by senior rate

What is the LOTO pre isolation procedure?
1) Obtain authorisation
2) Identify all isolation points
3) Inform users and anyone affected by the supply
4) Complete LOTO index sheet, obtain serial number
5) Complete relevant sections of LOTO safety certificate
6) Complete LOTO label
7) Turn any equipment off if possible.
8) Remove fuses and attach part A of label to empty fuse holder
9) Part B of LOTO label on LOTO board/ Part C with fuses in box provided
Working on dead equipment
1) Isolate and carry out the correct LOTO tag out procedure
2) Prove it is dead by using an item of GPTME
3) Carry out a voltage test between phases
4) Carry out a voltage test between each phase and a known good earth
5) Discharge any capacitors in the circuit
6) Fit shorting strips to prevent capacitors from recharging
Correct electric shock procedure
1) Raise alarm immediately
2) Isolate the supply by switching off supply or open breakers
3) If supply cannot be isolated then stand on dry insulating material and push and pull with a rescue hook
4) Check for response
5) Shout for help or press fire alarm call point
6) Open the airway and check for normal breathing
7) Keep shouting for assistance until help arrives
8) Carry out CPR until qualified help arrives

Why do we use long term LOTO?
1) For equipment that is likely to be out of action for a period in excess of 6 months
2) Equipment which is redundant and needs authority or guidance for removal
3) Electrical supply has been fitted but not equipment installed
4) When electrical equipment fitted for a specific task or trial has been removed
5) Short term LOTO that is in excess of 6 months and still needs to be rectified
