Thoracic and Lumbar Flexion
o 4 inches
o Standing or sitting
o Starting: C7 to S1
o Have pt. bend forward
o Ending: Measure C7 to S1 and calculate difference.
o Stabilization: the pelvis and hip joints are stabilized.
Thoracic and Lumbar Extension
2 inches
o Same positioning, measuring, and stabilization as flexion
Thoracic and lumbar lateral flexion
Compare with opposite side – much variability
o Standing with feet flat on floor 2 inches apart or supine with medial malleoli
touching
o Starting: tip of middle finger to floor directly underneath or tip of middle
finger to lateral malleolus
o Subject bends to one side with as little rotation as possible.
o Ending: take measurement and find difference
o Stabilization: the pelvis is stabilized.
Thoracic and Lumbar Rotation
Sitting with feet flat on floor and spine erect (no back support), arms across
chest
o No quantitative measure taken.
o Observe and compare to other side.
o Stabilization: the pelvis is stabilized.
Upper Rectus Abdominis
Supine
o 0 – no contraction
o 1 – contraction, no thorax depression
2- contraction with partial thorax depression
o 2 – contraction, full thorax depression
2+ - upper limbs at side of trunk, trunk begins motion against gravity
3- arms by side spines of scap clear table
o 3 – arms by side, inferior scapular angle clears
3+ - arms straight ahead, inferior angles clear
o 4 – arms ‘x’ chest, inferior scapular angle clears
4+ hands placed behind head elbows forward, inferior angles clear table
o 5 – hands on head, shoulders abducted, inferior scapular angle clears
Lower Rectus Abdominis
o Supine
o Keep one hand on lumbar spine – test stops when lordosis exaggerates
o Keep one hand ready to catch leg
External and Internal Obliques
Supine, legs straight
o 0 – no contraction
o 1 – contraction, no movement (rotation)
2- partial rotation
o 2 – full rotation
2+- arms by side, elevation of opposite scap
3- arms by side, elevation of half of scap
o 3 – arms relaxed, inferior scapular angle clears
3+- arms straight in front, op cap clears and same side cap begins to clear
o 4 – arms ‘x’ chest, inferior scapular angle clears, part of upper scapular angle
clears
4+- upper limbs behind neck, both caps clear
o 5 – hands on head, shoulders abducted, both scapula clear
o Stabilize pelvis and shoulder of EO
Rectus Abdominis
Inferior Attachment: pubic symphysis, pubic crest
Superior Attachment: xiphoid process and 5-7 costal cartilage
A: flex vert column, asst in posterior pelvic tilt
External Oblique
PA: external surfaces 5-12 ribs
DA: anterior iliac crest, linea alba, pubic tubercle
A: cl rotation of trunk, compress abdominal viscera
Internal Oblique
PA: Thoracolumbar fascia, anterior 2/3 iliac crest, inguinal line
DA: inferior border rib 10-12, linea alba, pectin pubis
A: Ipsi rotation of trunk
