GH: Distraction
Supine
Arm in resting position (or approximating restricted ROM)
Stand facing client. Grip proximal humerus close to axilla, from medial and lateral side
Distract lateral, anterior, and inferior (perpendicular to joint surface)
GH: Lateral Distraction
Supine
Arm beside body
Stand facing client
Place hand on medial humerus, by axilla, forearm against ribcage. Stabilize with other arm.
Distract laterally using forearm against ribs.
GH: Inferior Glide
Abduction
Arm in resting (or approximating restricted ROM)
Stand at head of client. Mobilizing hand at proximal humerus. Support arm with other hand.
Traction, then distract inferiorly, using legs and hips to generate movement.
GH: Posterior Glide
Flexion, internal rotation, horizontal adduction
Supine. Angle slightly obliquely, or place pillow under shoulder.
Mobilizing hand at proximal humerus. Other hand supports distal humerus.
Distract, then glide posteriorly
GH: Anterior Glide
External rotation, extension, horizontal abduction
Supine. Arm in resting position.
Stand at side of client, facing GH joint. Mobilizing hand under proximal humerus. Other hand supporting arm
Traction, then glide anteriorly. Direct with supporting hand.
(Can also be done prone)
Humero-Ulnar Distraction
Supine. Upper arm resting on table, elbow positioned off table, forearm on clinician’s shoulder
Stand (or sit) at hip, facing elbow.
Stabilizing hand grips anterior humerus; mobilizing hand grips proximal ulna. (Or both hands on prox forearm) Don’t squeeze too hard.
J glide: posterior, inferior

Supine
Stand at side, facing elbow
Stabilizing hand holds distal anterior humerus; mobilizing hand grips distal radius
Distract perpendicular to joint surface
Humero-Radial Compression
Supine. Elbow joint at 90º flexion, 5º supinated, wrist extended
Stand at side, facing elbow
Stabilizing hand supports distal humerus; mobilizing hand grips palm of hand
Compress downward

Extension
Supine, shoulder medially rotated so radial head on top
Stand at side, facing elbow. Grip from side.

Sitting or supine. Forearm supported by pillow.
Hold on either side of joint line (thumbs posterior, second fingers anterior). Thumbs and middle fingers on either side of joint lines.
To distract, bring index fingers between middle fingers.

Forearm pronated and supported by pillow.

Control calf between bicep and torso; stabilizing hand beneath distal thigh/knee; mobilizing hand on proximal hamstrings.

Prone, angled, so knee slightly off table, pillow under treatment side hip, shins
“Figure 4” leg.
From opposite side of table, stabilize above knee, mobilizing hand at greater trochanter,

Acetabular Femoral: Postero-lateral glide
Internal rotation
Prone. Knee flexed
Find joint line in between ischial tuberosity and greater trochanter. Move just medial, by acetabular rim. Use PIPs. Take time.
Stabilizing hand on ankle.
Glide acetabulum anterior, which means femoral head glides postero-lateral
Can be sensitive

Adduction, medial rotation
Supine
Knee over shoulder, hands linked at mid-upper thigh, at 9:00.

Tibial-Femoral Joint: Distraction
Seated. Pillow under distal thigh
Grip distal tibia from medial and lateral sides
Distract inferiorly and a little anteriorly
Tibial-femoral: Posterior glide
Flexion
Supine. Leg bent. Foot stabilized.
Grip proximal tibia with both hands, with webspace anterior (chokehold); or stabilize under femur with one hand and mobilize proximal tibia
Glide posteriorly
Tibial Femoral: Anterior Glide
Extension
Supine. Leg bent. Foot stabilized (knee & pillow).
Grip proximal tibia with both hands, thumbs anterior, fingers posterior
Glide anteriorly, monitoring joint line with thumbs

Supporting hand holds medial distal femur; mobilizing hands holds lateral proximal tibfib

Supine. Stand on lateral side of leg. Knee flexed.
Stabilizing hand holding medial distal lower leg, mobilizing hand grips lateral side of joint line.
Gap medially.
Tibial Femoral: Lateral Gap
Flexion, extension, genu valgum
Supine. Stand on lateral side of leg. Knee flexed.
Stabilizing hand holding lateral distal lower leg, mobilizing hand grips medial side of joint line.
Gap laterally.
Prox TibFib: Posterior Glide
Supine. Knee supported by pillow. Medially rotate the leg to bring fibular head more anterior.
Stabilizing hand holds medial/posterior proximal tibia. Heel of mobilizing hand on anterior surface of fibular head.
Glide posteriorly
Prox Tibfib: Anterior Glide
Prone. Lower leg supported by pillow.
Stabilizing hand holds proximal medial tibia, heel of mobilizing hand on posterior fibular head.
