G.O. 108.13 EMS GFD Quality improvement plan
The goal in quality improvement is?
to create an atmosphere in which problems are prevented and process is improved to maximize customer’s satisfaction.
Who is responsible for the oversight and supervision of the entire CQI process and communicating with all the EMS participants.

What is the purpose of Continuous Quality Improvement (CQI)
The purpose is to identify root causes of problems or trends, intervene to reduce or eliminate these causes, and take steps to make improvements for the future. It is a system that has the ability to measure and also prevent future concerns.
EMS Chief Officer Responsibilities include:
- Provide oversight of the CQI Committee (a subcommittee of the EMS Committee)
- Review individual QI Reports and take necessary action.
- Provide and be an access point for Internal and External Customers as identified in Section IIIF.
- Create a Review Process/Panel, as needed, to review policy, procedures, proposed training, and potential discipline as a result of charts reviewed in the CQI process. This is an internal body, not intended to supersede any state, city, or department guidelines and requirements.
CQI Analyst/EMS Coordinator responsibilities include:
- Implement, monitor, and evaluate the CQI System, including requirements described in (I).
- Monitor quality indicators via database analysis.
- Review and participate in research generated by the CQI process.
- Forward CQI recommendations to EMS OCP (Organizational Cooperative Process) Committee.
- Manage EMS database to assure quality and completeness of databases.
- Provide regular CQI reports to the Medical Director, Base Hospital CQI coordinators, CQI Committee, and GFD Staff Meetings.

What are the medic rides designed for?
Medic units are designed to return Advanced Life Support (ALS) engine companies to an available status as soon as possible. Medic units are not automatically dispatched on ALS incidents unless they are the closest unit and should add themselves to all incidents in the City of Glendale within five miles of their location.

Medic units are required to monitor radio traffic and carry unit pagers to insure that all calls for service are acknowledged. Medic 155 and 158 should function as adaptive response units within the City of Glendale and be available to cover unprotected areas or stations. M157 should stay centrally located with the city unless otherwise directed by the Shift Commander.
What is the policy for the medic ride going into other cities?
Medic units that are dispatched into adjoining automatic aid agencies may be cancelled by the units from the responsible jurisdiction. If the medic unit is not cancelled they will provide the same service as within our city per the Phoenix Regional Automatic Aid Agreement. Medic units should not self dispatch into adjoining jurisdictions and should only respond if special called by a Glendale Engine Company, Ladder Company, Battalion Chief, or Deputy Chief.

What are our Code 3 transport policy?
Glendale Fire Department values the safety of all emergency response personnel. We will insure that Code 3 transports occur in the safest manner. Two personnel should be placed in the cab area of all rescues for Code 3 transports whenever possible. The second crew member in the cab will function as a traffic spotter to assist the driver during the Code 3 transport
What is the rule of Arizona Department of Health Services when a Captain elects to have both Southwest Ambulance members placed in the cab area during a Code 3 transport?
If a Captain the attending Glendale Fire Department paramedic will be required to assist the Southwest Ambulance crew with patient documentation in order to comply with Arizona Department of Health Services Rules.
