Ischemic Stroke Flashcards
what are Barrys Signs and symptoms
- Difficulty walking
- Collapsed
- Slurred speech
- Facial droop
- Difficult arm movements
- Numbness in arm, leg and face
Identify Barrys risk factors that contributed to his stroke.
- Advanced age (being older than 50)
- Hypertension poorly controlled
- TTDM
- Male
- Previous stroke few years back.
Compare Ischemic and Hemorrhagic stroke
Ischemic
1. Causes by thrombus or embolus
2. Narrowing of cerebral arteries causing tissue ischemia
3. Tissue infarction if blood supply does not return quickly
Hemorrhagic
1. caused by rupture of cerebral artery
2. bleed and hematoma forms around tissues
3. tissue damaged and increased inter-cranial pressure
Explain the pathophysiology of an ishcemic stroke
- multiple emboli likely due to atherosclerosis plaque rupture
- Blocks the left cerebral artery
- results in tissue ischemia and hypoxia and will result in infarction if blood supply not supplied in time
- Inflammation and neuronal necrosis
Describe the associated complication of a stroke
- Left cerebral damages cause function loss to the Right side of body.
Speech: Dysarthria lack of motor control.
Dysphagia
Hemiparesis: Decreased motor function, Truncal ataxia and R) Sensoral loss
Vision: R) Homonymous Hemianopia and Decreased visual co-ordination
Fatigue
describe the three tests carried in the Glasgow coma scale
- Eye opening: indicates the function of brains activating centers
- Verbal: The ability for patient to understand communication
- Motor: Ability to obey command
Lowest score can be 3 (worst)
Highest score 15 (best)
Barry initially scored 10/15
Explain the different diagnostic tests used throughout barrys hospitalisation
CT scan: Shows the precise differences in densities and highlights location of hematoma and strokes. which is quick for early detection
MRI: Scans more in depth and confirms infarct brainstem
Describe the acute medications given
rtPA: ONLY GIVEN FOR ISCHEMIC STROKE
1. Plasmin is a fibrinolytic that breaks down fibrin holding blood clots together.
2. allowing reperfusion of the cerebral vessels
What are some long term medications used to manage stroke
Anti - Coagulation - Aspirin
- Blocks prostaglandin synthetase action and is a
platelet aggression inhibitor
