ISBAR (short answer, application question, write out the ISBAR)
A
- Identify (I): Establish clear communication and avoid errors
o Name, professional position, and location (e.g., “I am a nurse at the ICU at General Hospital”).
o Confirm who you are speaking to (e.g., the on-call physician)
o Identify the patient by name and unique ID number to prevent mix-ups. - Situation (S): Reason for your call to focus the listener.
* Example: “I am calling about Mr. John Doe in room 215. He is experiencing increased shortness of breath and his oxygen saturation has dropped to 88%.” - Background (B): Provide only relevant and concise background information to give context.
o History: Recent surgeries, conditions, or key events.
o Assessment Findings: Vital signs, lab results, or test findings.
o Current Management: What has already been done or prescribed. - Assessment (A)
* Purpose: Share your professional interpretation of the situation.
o State what you think is happening (e.g., “I suspect the patient may be developing sepsis based on a fever of 39°C, tachycardia, and low blood pressure”). - Recommendations (R): Request specific actions or clarify next steps.
o State what you need from the listener (e.g., “I recommend starting IV fluids and ordering a chest X-ray”).
o Ask questions to confirm mutual understanding (e.g., “Would you like me to monitor the patient’s vitals hourly?”).
