What is IBS
Chronic condition of the bowel (irritation of the bowels)
IBS is more common in what gender and what age?
Women
20-30yrs
Symptoms of IBS
ABCD
abdominal pain
Bloating
Constipation
Diarrhoea
Flatulence, mucus from bottom, lethargy, bowel incontinence
Which conditions have the symptoms ABCD (abdominal pain, bloating, constipation, diarrhoea)
IBS
coeliac disease
Non drug treatment of IBS
Limit fresh fruit
Eat regularly
Increase SOLUBLE fibre because easily digested
Increase water - 8 cups daily at least
Reduce caffeine, fizzy, alcohol
Examples of soluble fibre
Oats
Sterculia
Isphagula husk
Which bulk forming laxative would you avoid in IBS and why
BRAN
Hard to digest hence exacerbates symptoms
Which osmotic laxative would you avoid in IBS and why
Lactulose because exacerbates bloating
Drug treatments in IBS
CRAMPS:
Antispasmodics - buscopan, peppermint oil, mebeverine
Antimuscarinics - atropine, trospium, oxybutynin (increases constipation risk)
CONSTIPATION:
Laxatives but avoid bran and lactulose
Linactolide - severe IBS associated with constipation
BLOATING:
Peppermint oil
DIARRHOEA:
Loperamide
Patients not responding to treatment (antispasmodics, loperamide, laxatives) in IBS
offer:
TCA - amitriptyline
SSRI - fluoxetine
CBT- if stress is a trigger of IBS
Whats offered in severe IBS associated with constipation to relieve constipation
