A
In 2000, outbreak of e-coli in the water plant due to negligence and lack of knowledge of the managers. They were grandfathered into the management positions without being required to have the training. Resulted in 6 deaths.

A
They created and used a device that cheated the emission tests. Resulted in the cars emitting 40x the amount of nitrous oxide allowed in the US.

Boot camps for youth
A
Similar to military training camps. They don’t offer therapeutic programs to help youth who have emotional, behavioural, and mental problems. Not effective and does more harm than good.
