What should inventories include?
All on hand controlled substances including expired
Who needs to sign the inventory?
Person taking inventory and PIC should sign and date
Signature of PIC and date of the inventory shall be motorized within 72 hours or 3 working days for all inventories except initial and change in PIC
Counts Required
Exact count for C2
Estimated count for C3, C4, C5 if the container has fewer than 1000 tablets
What inventories are required?
Initial inventory
Annual inventory per TSBP
Change in Ownership for Class A, A-S, C, C-S or F Pharmacy (on the date of change as the closing inventory for the seller and initial inventory for buyer, and C2a have to be transferred using a DEA 222)
Change in PIC for Class A, A-S, C, C-S or F Pharmacy (not notarized)
Who must maintain perpetual inventories?**
Class C and C-S for C2
Class C Ambulatory Car Facility and Class F for all controls
All pharmacies for controls stored at a remote location
Before Closing a Pharmacy
14 days prior PIC must:
- Notify DEA
- Post a closing note informing patients of where to acquire records
Closing day, the PIC must:
- Take a closing inventory
- Transfer prescription drug order files and patient medication records
After closing, the PIC must
Within 10 days:
- PIC must provide to the Board written notice of the actual date of closing, the pharmacy license, a statement that an inventory was conducted, the manner the drugs were transferred or disposed, name and address of the pharmacy to which records were transferred
