SBAR format
Military format that is the standardized format for case presenting in hospital
Audience, setting and clinical situation dependent
Modulate pacing and good eye

S: Situation: (synopsis of what is going on usually includes diagnosis and symptoms)
B: Background: (mostly history of the patient and lab orders. SAMPLE location)
A: Assessment: (results of the tests ordered and responses to intervention by patient)
R: Recommendation: (what should happen next)
Ask questions if needed

S: Signs and symptoms A: Allergies M: Medications P: Past history L: Last oral intake E: events leading up to presentation
Transitioning patients between physicians
Most common site of when patient injury occurs. Important that Oral cases are presented in a clear standardized form so that each physician understands what’s going on.
Signpost method
A derivative of the SBAR model
Use open ended responses and only share pertinent information.
Allows for more time for the recipient to ask questions about what they want to know
