What are the three main parts of the fetal head?
Face
Base of skull
Vault > frontal, parietal, and occipital, “the roof”, these are soft bones so that they can overlap
What causes cervical dilation?
Longitudinal muscle fiber pulled over presenting part
Hydrostatic pressure of the fetal membranes
What is the fetal attitude and what is the normal?
Relation of fetal body parts to one another
Normal- general flexion
What’s the narrowest diameter through which the fetus must pass?
The maternal Ischial spine
** Ischial spine is the landmark designated as “zero station”
What can happen if cervix is not completely dilated but mom is bearing down?
Cervical edema
Possible tearing and bruising of cervix
Maternal exhaustion
What is the primary force in the first stage of labor?
Uterine muscle contraction
What is the secondary force during the second stage of labor?
Abdominal muscles to push
What are the signs of true labor?
Dilation and effacement
Contractions regular and increasing in frequency, direction, and intensity
Pain in the back that radiates around to abdomen
Pain not relieved by ambulation or resting
When is PPROM?
Before 37weeks
Where is the lumbar epidural block inserted?
Where is the spinal block inserted?
Between Dura mater and ligamentum flavum
