Verify, Explain, Hand hygiene
Verify the written order prescription for iv therapy, check iv fluid and other devices,
Explain the procedure to reassure the client, and observed 10 R’s
Do hand hygiene before and after the procedure
Choose site for IV
Apply
Apply tourniquet 5 to 10 cm (2- inches) above injection site depending on condition of the patient
Check for
Check for radial pulse below the tourniquet
Prepare site
Prepare site with effective topical antiseptic according to the hospital policy, or cotton balls soaked with alcohol in circular motion and allow it to dry for 30 seconds.
Using the appropriate
Using the appropriate IV cannula, pierce the kin with needle positioned on a 15 - 30 degree angle
Upon
Upon flashback visualization decrease the angle, advance the catheter and stylet into the vein, check the tip of catheter can be rotated freely inside the vein
Position
Position the IV catheter parallel to the skin. Hold stylet stationary and slowly advance the catheter until the hub is 1mm to the puncture site.
Slip a sterile gauze
Slip a sterile gauze under the hub. Release the technique, remove the stylet while applying digital pressure over the catheter with one finger about1 - 2 inches from the tip of the inserted site
Connect
Connect the infusion tubing of the prepared IVF aseptically to the IVF catheter
Open the clamp
Open the roller clamp, regulate the flow rate and reassure the client
Anchor the needle firmly in place with the use of
Transpore for dressing directly to the puncture site
Tape for anchoring
Tape
Tape a small loop of tubing for additional anchoring, apply splint if needed.
Calibrate
Calibrate the IvF bottle and regulate flow of infusion according to the prescribe duration.
Label on
Label on tape near the iv site to indicate the date of insertion, type and gauge of iv cathter and countersign
Label with plaster
Label with plaster on the iv tubing to indicate the date when to change the iv tubing
