High dose ICS:
Can increase risk of Pneumonia : should be used with caution in those with Hx of Pneumonia.
Use with caution in children with risk of growth suppression.
MOA

Pro-inflammatory proteins are down-regulated.
Anti-inflammatory proteins are up-regulated.

1) reduced mucosal inflammation
2) reduced mucus production
= reduced exacerbations + improved symptoms.
