inhalation sedation 2 Flashcards
What equipment is needed for inhalation sedation
- Gas cylinder of oxygen (white)
- Gas cylinder of nitrous oxide (blue)
- 2 pipes one for oxygen one for nitrous oxide
- Spare gas cylinders for oxygen and nitrous oxide
What is the average flow rate of nitrous oxide we give to an adult
6L / Min
What is active scavenging
Actively removing residual nitrous oxide
At what rate do we actively scavenge
45L/ Min
Name the 2 different types of nasal masks
- Autoclavable
- Single use
List some of the safety features used during inhalation sedation
- Colour coding gas tubes
- pin index system
- Minimum oxygen of 30%
- Oxygen flush
- Oxygen fail safe
- Reservoir bag
- Air trap valve
- Active scavenging
What is an oxygen flush and when is it delivered
Allows for 100% oxygen to be delivered in case of emergency
What is an oxygen fail safe
Ensures 100% nitrous oxide cannot be delivered
What does active scavenging help do
Reduce trace levels of nitrous oxide in the atmosphere
What does the pin index system ensure
Oxygen and nitrous oxide are connected to the correct fixings
What does a reservoir bad do
Assists in monitoring the patients breathing
What does the air entrainment valve allow for
The flow of atmospheric air into the system should the gases run out
How often is the inhalation sedation equipment serviced
Every 6-12 months
When should you check the equipment
Daily checks before use
Prior to doing the inhalation sedation what can we do with the patient
- Show them the nasal mask and let them try I ton
- Consent prior to the appoint
- Give pre op and post instructions
What do patients need to consent to prior to IHS
Dental treatment and sedation
What do we need to do and what information do we need to gather in the pre sedation assessment
- Full medical history
- Allergies
- Airway and nasal pathency
- BMI
- ASA Status
- Dental history
- Anxiety assessment
- Sedation and GA history
- Social history
- dental treatment plan and radiographs
- Which sedation we are opting for
- Any requests from the patients
What pre operative instructions do we need to give to the patient
- Light meal 2 hours before appt
- No alcohol 24 hours prior
- Children to be accompanied by a responsible adult (no other children)
- Transport home in car or taxi
- Avoid bikes, trampolines, machinery and driving
- Child should be accompanied by an adult after appt
Does an adult need an escort following IHS
Not according to SDCEP unless the adult has other indications such as mobility issues
What is IHS described as
A psycho pharmacological sedation
What do we need to of to ensure the dental treatment during iHS is successful
- Good behaviour management
- Good rapport with patient
- Consider an intro appt with no treatment
- Stop signals
- Gain trust
- Effective LA including topical
What pre op checks do we need to Carry out
- Check we have the correct staff: operator, nurse and runner nurse
- Check equipment
What pre op checks do we need to carry out on the patients
- Patient identification
- Written consent
- Patients nose
- When they had their last meal and alcoholic drink
- MH
- Pregnancy status of both patients and carer
- For a child is there a responsible adult with them
- Transport arrangements
Talk through the delivery of nitrous oxide for IHS
- Fit the appropriate mask and make pt comfortable
- deliver 100% oxygen
- Adjust flow rate to 4-6L/min
- Observe reservoir bad it should expand with each breath
- After a few mins begin to titrate nitrous oxide in 10% increments per min
- Once you reach 30% nitrous oxide start increasing in 5% increments
