Definition of inflammatory bowel disease?
Its name for different types of diseases causing inflammation of GI tract:
Ulcerative Colitis
Crohns Disease
Main features of crohns?
N: No blood or mucus
E: Entire GI tract
S: Skip lesions on endoscopy
T: Terminal illeum most affected
S: Smoking is risk factor
Main features of Ulcerative colitis features?
C: Continuous inflammation
L: Limited to colon + Rectum
O: Only superficial Mucosa affected
S: Smoking is protective
E: Excrete blood + mucus
U: Use aminosalicylates
P: Primary Sclerosing Cholangitis
How to test for inflammatory bowel disease?
Routine bloods for anaemia, infection, thyroid function.
CRP indicates inflammation and active disease
Faecal Calprotectin
Endoscopy
Imaging with ultrasound, CT and MRI.
Management of Crohns?
First line = Steroids ( oral predisolone)
Surgery should be done if it effects the distal ileum
Management of ulcerative colitis?
First line = Introduce remission
If severe IV corticosteroids
Surgery is possible but patient will be left with a J-pouch
Ulcerative colitis definition?
Where ulcers form across lumen of large intestine because of antibodies destroying mucosa. ( autoimmune disease)
Crohns disease?
Where body has bacteria which triggers a unregulated inflammation response which destroys cells as well as bacteria. They end up greeting granulomas which destroy cells and ulcers which deeper than just mucosa.
How does Crohns appear?
Crohns has a cobblestone appearance.
Where does crohns affecct on body?
Right lower quadrant ( illium )
Where does Ulcerative Colitis affect on body?
Left lower quadrant ( rectum + colon )
