Infection of lung parenchyma characterised by consolidation
Common pathogens causing CAP
Streptococcus pneumonaie, Haemophilus influenzae, Mycoplasma pneumoniae
Common pathogens causing HAP
Enterobacteria, Staphylococcus aureus
Symptoms of pneumonia
SoB, Productive cough, Haemoptysis, Stabbing chest pain (worse on inhalation), Anorexia, Malaise
Signs of pneumonia
Fever, Tachypnoea, Tachycardia, Hypotension, Decreased chest expansion, Dull percussion, Increased vocal resonance, Pleural rub
Investigations for pneumonia
Bloods: FBC, U+Es, LFTs, Culture
CXR, Sputum culture, ABG, Atypical screen

Management of CURB65 1 pneumonia
5 day course of Amoxicillin 500mgs TDs
Management of CURB65 2 pneumonia
5 day course of Amoxicillin 1g TDs and Doxycycline 200mg OD
Management of CURB65 3+ pneumonia
5 day course of Co-amoxiclav 1.2g TDs and Doxycycline 200mg OD
Causes of non-resolving pneumonia
Empyema, Abscess, Immunocompromisation, Wrong ABx, Atypical pathogen, Cancer, PE
Symptoms of TB
Productive cough, Haemoptysis, Chest pain (worse on inhaling), Malaise, Weight loss, Night sweats, Effusion
Signs of TB
Tachycardia, Fever
Investigations for TB
Bloods: FBC, U+Es, LFTs
CXR, Sputum culture, HIV test, Vitamin D test, Quantiferon
Management of TB
Conservative: Barrier nursing, Move to a side room
Medical: RIPE
