EGD
S/s of infectious esophagitis
Odynophagia and dysphasia, oral thrush
Treatment plan for candida infectious esophagitis
Oral fluconazole 200-400mg x1 then 100-200mg po QD x14-21 days
Refractory infectious esophagitis treatment
Voriconazole 200mg BID x14 then daily x7 days after symptoms disappear
Can do micafungin if unable to swallow
S/s of herpetic infectious esophagitis
Vesicles on the nose and lips
What is the diagnostics for herpetic infectious esophagitis
EGD: vesicles and small punched out ulcerations, biopsies w/ ground glass nuclei, eosinophilic Cowdry’s type A inclusion bodies, and giant cells
Treatment plan for herpetic infectious esophagitis
Immunocompetent: Acyclovir 200mg 5x qd x 7-10days
Immunocompromised: Acyclovir 400mg 5x qd 14-21 days
Alternative therapies: Famciclovir 500mg po TID or Valcyclovir 1g po TID
Or Acyclovir IV for severe cases
What is the diagnostic for CMV infectious esophagitis
EGD: serpiginous ulcers
Tx plan for CMV esophagitis
Ganciclovir and Valganciclovir for 3-6 weeks until healed and may need maintenance therapy
S/s of Infectious esophagitis
Odynophagia: most common
Dysphasia
Chest pain
Hemorrhage
