What types of toxins are A and B in c. Diff
A: Enterotoxins
B: cytotoxin
Causes of c. Diff
Acquired exogenously
Bacteria carried in stool
Exposure to anti microbial agents-then toxin- then inadequate immune response
Risk factors for c. Diff
Age, severity of underlying issues, GI sx, rectal thermometers, enteral tube feed, antacid treatment, antibiotics
S/s of c diff
Diarrhea
Fever, ABD pain, leukocytosis
A dynamic illeus: high risk for toxic megacolon and sepsis
Fulminant cdiff: hypotension, shock
Diagnosis and treatment of c diff include ABX taper
Diarrhea >3/24h for >2 days
Positive stool culture
D/C ABX
Hydration
Oral vanco or difficid
125mg po q6h x10days — 125 mg po BID x7 days — 125mg po qd x7 days — 125mg po qd q2-3 days x2-8 days
Treatment for fulminant c diff
Oral Vanco 500mg po q6 plus Flagyl 500mg IV q8h
Consider vanco enema
May need fecal transplant
