Infections of the NS Flashcards
Which compartment is meningitis in?
Subarachnoid
What is the cell responsible for innate immunity in the CNS?
Microglia
Vasogenic edema
Swelling in the brain from an accumulation of fluid
Its the end point of a response to infection
Can cause neuronal injury, seizures, ischemia
4 main symptoms of meningitis
Fever
Altered mental status
Neck stiffness (nuchal rigidity)
Headache
The petechial/purpuritic rash is due to what bacteria?
N meningiditis
Workup for meningitis
CBC, chemistries, coagulation, blood cultures (stat)
CXR
CT head
LP (dont have to wait to give the antibiotics for this)
CSF analysis
Cell count Protein Glucose Gram stain, culture Viral PCR Fungal/mycobacterial culture
CSF findings for bacterial
- Pressure
- WBC
- Protein
- Glucose
- Increased
- Very high, mostly neutrophils
- 10x increased normal
- Decreased
CSF findings for viral
- Pressure
- WBC
- Protein
- Glucose
- Normal/increased
- Increased
- Normal/increased
- Normal
CSF findings for fungal
- Pressure
- WBC
- Protein
- Glucose
- Increased
- Increased, mostly lymphocytes
- 2-3x increased normal
- Normal/decreased
4 high risk features that should make you do a CT before getting an LP
Immunocompromised
Depressed LOC/focally abnormal neuro exam
Seizures
Known mass lesion
Initial drug management for suspected bacterial meningitis
Ceftriaxone
Vancomycin
Dexamethasone
Why are we giving vancomycin?
For resistant streptococcus
Not for MRSA
3 main complications of meningococcal meningitis
DIC
Severe sepsis/mortality
Permanent hearing loss
Treatment of viral meningitis
Acyclovir
