Infections above lungs Flashcards
severe sore throat, stridor, fever, dysphagia, drooling, tripod position
NO pharyngitis
Epiglottitis
Epiglottitis is primarily caused by
Haemophilus Type B (G -)
I’m not sure if this pt has epiglottitis or not. I should order ____
lateral neck XR to look for thumbprint sign (enlarged epiglottis)
Epiglottitis Treatment
- Ceftriaxone (Rocephin) for pt
- Rifampin for household contacts
- Report to CDC
6 month old - 3 yr old with cold and barking cough. symptoms worse at night
Croup
Order ___ to rule out Croup
check for ___ sign
neck or AP Chest XR
check for steeple sign
Croup Trmnt
- cool air +/- corticosteroids
- albuterol if wheezing
sometimes nebulized racemic epinepherine
Influenza season
WINTER
Parainfluenza season
FALL
Stridor is heard in the ____; wheezing is heard in the ____
Stridor = neck
Wheezing = lungs
3 weeks -16yo with URI that progresses to HIGHER fever, cough, and inspiratory stridor, and slight respiratory distress
Bacterial Tracheitis
Bacterial Tracheitis Trmnt
- Airway Managment
- IV ABX: Rocephin + (oxacillin or nafcillin) OR single trmnt clindamycin for community acquired MRSA.
- Vanco if pt appears toxic
staph aureus and group A beta-hemolytic strep (both G +)
Croup Etiology & seasonal
Parainfluenza type 1,2, &3 virus, influenza A & B, adenoviruses
FALL
Croup affects what part of the throat?
larynx
order ____ to rule out bacterial tracheitis
XR for steeple sign at trachea
pt with severe sore throat, fever, stridor, and NO pharyngitis. Concern for?
Epiglottitis
Baby has nasally inspiratory stridor but no signs of infx?
Laryngomalacia
A collapse of supraglottic structures during inspiration
Most common congenital stridor
laryngomalacia
A collapse of supraglottic structures during inspiration
Child has an URI that progressed to HIGHER fever, cough, and inspiratory stridor. They keep tossing and turning (cant get comfortable). No drooling
Bacterial Tracheitis
need hosp for IV ABX and airway managment
