waist height on surface, click closure once to temporary lock and twice to permanently lock, label has date assembled, date closed and department

stop what doing, take off gloves, get supervising clinician and explain situation, investigate injury site, if not injury them dispose of sharp and start again with new sharp

someone else fills it out, information confidential, not in notes, must be destroyed after occupational health called, students take blood samples at OH not GDH
