what is psychogenic stress?
fainting, palpitations, cold sweat, restlessness, excitation, trembling, weakness
what is the cause of psychogenic stress
lack of oxygenated blood to brain
what are the clinical features of psychogenic stress
light headedness, pallor, beads of sweat, bradycardia, nausea, pupil dilation
how do you manage psychogenic stress
lay flat with legs raised, loose clothing, ventilate room, glucose in a sweet drink
what drugs can adrenaline interact with?
tri-cyclics, B blockers, diuretics without K, cocaine
what are most allergies for?
latex bung/preservatives/antioxidants
what is included with toxicity
convulsions, loss of consciousness, respiratory depression, circulatory collapse
what happens to pregnant women if give felypressin
induces labour
what is the effect of adrenaline on the heart
increase rate, force, output and excitability
what is the effect of adrenaline on blood vessels
coronary dilation, skin contraction, muscle dilation
what is the effect of adrenaline on blood pressure
systolic up and diastolic down but not much effect really
what is the effect of adrenaline on the lungs
bronchial muscle relaxation
what are the dangers of adrenaline?
cardiovascular disease, hyperthyroidism, phaeochromocytoma, drug interactions
what are the local complications of LA?
failure to achieve LA, prolonged LA, pain during or after injection, trismus, haematoma, intravascular injection, blanching, facial paresis, broken needle, infection, soft tissue damage, contamination
how does prolonged anaesthesia occur?
direct trauma from needle, multiple passes with same needle, chemical trauma from direct injection
how would a patient have pain during or after injecton?
giving injection too fast or patient has muscular spasm during injection
when does trismus present
within a few hours of IDB and may restrict opening of mouth lasting weeks or months
what is the cause of trismus
injection to medial pterygoid
how do you manage trismus?
reassurance, muscle relaxant, anti-inflammatory
how do you tell if facial palsy is stroke or due to LA?
upper spares upper for stroke (can wrinkle forehead) whereas LA you can’t do anything
when does facial palsy present?
unilateral motor nerve paralysis within minutes of IDB
