Chronic dilatation of one or more bronchi
Bronchi exhibit poor mucus clearance
Predisposes to infection
What is the gold standard diagnostic test for bronchiectasis?
High resolution CT
Name some causes for bronchiectasis
Post infective: whooping cough, TB
Immune deficiency: Hypogammaglobulinaemia
Genetic/mucociliary defects: CF, youngs, kartageners
obstruction
toxic insult
allergic bronchopulmonary aspergillosis
secondary immune deficiency eg hiv, malignancy
RA
associated with IBD
What is Youngs syndrome?
Triad of bronchiectasis, sinusitis and reduced fertility
What is Kartageners syndrome?
Triad of bronchiectasis, sinusitis and situs inverses
What are some common causative organisms of bronchiectasis?
Haemophilus influenzae Pseudomonas aeruginosa Moraxella catarrhalis Stenotrophomonas maltophilia Fungi - aspergillus, candida Non tuberculous mycobacteria Less commonly staph aureus (CF)
How is bronchiectasis managed?
Treat underlying cause Physio for mucus clearance Antibiotics Supportive measures e.g. flu vaccine, bronchodilators Pulmonary rehab
