Adhesie van bacteriën aan beschadigde klep tijdens transiënte bacteriemie –> bacteriële groei in cardial letsel (vegetatie) –> lokale uitbreiding en cardiale schade
o Septische embolieën in huid, nieren, milt en hersenen
o Circulerende immuuncomplexen –> glomerulonefritis, artritis, vasculitis
o Embolisatie van vegetatieve fragmenten à infectie/ infarctatie van weefsel op afstand

• Septische arteriële embolieën –> focale infecties
• Osler noduli: pijnlijke, erythemateuze noduli meest frequent op vingers en tenen
• Janeway lesions: niet-pijnlijke erythemateuze nodulaire letsels meestal op handpalmen en voetzolen
• Splinterbloedingen: kleine, lineaire, longitudinale subunguale bloedingen
• Petechiën (zowel op huid als mucosa)
• Roth spots: witte spots op retina dicht bij optische discus met omliggende hemorragieën
• Septische embolieën: pijnlijke, hemorragische maculae, papels of nodules, meestal acraal
