What are the Class I indications for primary prevention for an ICD?
Primary Prevention: NYHA Class II-III, at least 40 days post-MI, LVEF </= 30%
What are the Class I indications for an ICD for secondary prevention?
Secondary prevention: patients with a history of SCA, VF or hemodynamically destabilizing VT.
What are the Class II indications for an ICD?
ScD HEFT patients: NYHA Class II-III, LVEF 30-35%; Ischemic and Non-Ischemic Patients, LVEF </= 30%
What are the CRT Class I indications?
LVEF 120ms, and Stable Optimal Drugs. Patients who also meet ICD indications are eligible for a CRT-D.
What are the four heart failure categories?
Class I, asymptomatic with ordinary activity, Class II, symptoms with moderate exercise, Class III, symptoms with minimal exercise, Class IV, symptoms at rest.
