Impulse Control Disorders
Predisposed inabilit to resist unplanned rapid reactions toward internal or external stimuli without regard for negative consequences
Intermittent Explosive Disorder
Recurrent outbursts of aggression that result in assault against people or property
Outbursts and aggression are out of proportion for triggering event or stressor.
These outbursts remit quickly.
More common in men than women.
Tx: SSRIs, Anticonvulsants, Lithium and propranolol.
Group/Family Therapy may be useful
Kleptomania
Inability to resist urges to stale objects that are not needed for personal use or monetary gains.
Pleasure or relief is felt while stealing. Often followed by intense guilt or shame.
More common in women
Increased likelyhood of comorbid mood disorder, eating disorders or OCD
Tx: Insight oriented psychotherapy and SSRI’s
Pathological Gambling
Persistent and maladaptimve gambling behavior as evidenced by five or more of the following.
Preoccupation w/ gambling Gamble with increasing amount of money Unsuccessful cutdown of gambling To relieve dysphoria Returning to reclaim losses lying about level of gambling Illegal acts to finance gambling Jeporidizing relationships for gambling Relying on others to financially support gambling
Tx: Group therapy (gamblers anonymous), Tx of comorbid mood disorders SSRI’s
Trichotillomania
Recurrent repetitive pulling out of ones hair causing visible hair loss
Tension is experienced before the pulling behavior and pleasure or relief occurs after.
Tx: SSRI, Antipsychotics, Lithium , Behavioral interventions such as hypnosis.
Pyromania
At least one episode of deliberate fire setting
Tension or arousal experienced before the act and pleasure, gratification or relief experienced when setting fires and subsequent consequences.
This is not for monetary gain, expression of anger or making a political statement.
Tx: behavior therapy, supervision, SSRI’s
