Behind glandular tissue, in front of muscle
Where are implants most commonly placed post-mastectomy?
Submuscular/Subpectoral to reduce risk of contracture
What type of implant is most common?
Single lumen saline
Capsular fibrosis:
additional thickening of normal scar tissue can occur do to fibrosis, leading to increased risk of contracture
How thick is a capsule during contracture?
> 1.5 mm, most common in subglandular implants
Glandular ptosis:
sagging/drooping of breast (s)
Herniation:
focal capsule break causing herniation of intact portion through the break
What 4 mammo views are needed to eval implants?
- CC limited compression
- MLO limited compression
- CC Eklund view
- MLO Eklund view
Propagation speed artifact is most commonly seen in
silicone implants (speed of sound moves slower in silicone than tissues)
Step Ladder Sign
U/S sign referring to the infolding of capsule layers in an intracapsular silicone implant rupture…correlates to MRI linguini sign
Linguini Sign:
MRI sign: when inner layer of the capsule ruptures it recoils and looks squiggly
Teardrop or Noose Sign:
non-collapsed silicone rupture with silicone trapped in a peripheral fold
Breast Implant-Associated Anaplastic Large Cell Lymphoma
disease associated with the fibrous capsule surrounding the implant..presenting as effusion around implant or as a mass
