What is a dental implant?
An artificial tooth that is surgically anchored into the jaw to hold a replacement tooth or teeth or a denture in place.
What are some advantages of a dental implant?
Do not rely on neighbouring teeth for support.
Control over the local anatomy, can rebuild what wasn’t there.
Screwed in place so will be stable in function and speech.
What the differences between an implant and a natural tooth?
No PDL in implants- screwed directly into the bone by osseo-integration.
Implant will not have any proprioceptors- no way of determining how hard you can bite.
Peri-implantitis instead of periodontitis.
Peri-mucositis instead of gingivitis.
What can an implant restore?
Single tooth
Multiple teeth
Secure in a denture firmly
Eyes, ears, hearing aids and noses.
What methods are used to retain the tooth within the implant?
Cement-retained or screw-retained.
What is an advantage of cement retained implants?
Better aesthetics.
What is a disadvantage of cement retained implants?
Difficult to get the tooth back off the implant once it is placed.
Cement can get stuck around the margins of the tooth- inflammation and bone loss.
What is an advantage of a screw-retained implant?
Implant is easily accessed later on to repair the implant in any way.
What is peri-implant mucositis?
Equivalent of gingivitis but around an implant.
Inflammatory lesion surrounding the peri-implant mucosa, with the absence of bone loss.
Plaque present around the implant, red gingivae, puffy.
What is peri-implantitis?
Equivalent of periodontitis but around an implant.
Bone loss.
Loose implant.
Suppuration
Pt senses a bad taste of smell.
What are common issues in implant cases?
Peri-implant mucositis
Peri-implantitis
Chipping of teeth- no proprioception
Implant into the maxillary sinus.
What is the role of a GDP with regards to implants?
OHI. diet advice, smoking cessation.
Assess OH.
Gently probe around the implant to see if there is any cemen, submucosal plaque or calculus.
Look for signs of inflammation
Remove supra-mucosal and sub-mucosal plaque and calculus.
PA of implants one year after they were placed.
Investigate and diagnosis suspected peri-mucositis and peri-implantitis.
What would you do if you detected someone has peri-implant mucositis?
Radiographic assessment to exclude peri-implantitis.
Supra and submucosal plaque and calculus removal.
Under what circumstances may an implant be provided on the NHS?
Oral cancer
Trauma
Developmental defects
Complete denture difficulties
