ages 20-60 at diagnosis

presentation
A
slowly progressive heart failure OR
acute decompensation

Gross pathology
A
heart:
- -enlarged
- -soft “flabby”
- -4 chamber dilatation
- -mural thrombi

Microscopic pathology
A
myocyte degeneration
myocyte atrophy
other myocytes w/ irregular hypertrophy
interstitial and perivascular fibrosis
**findings can be surprisingly minimal

Symptoms
A
nonspecific symptoms of HF

Signs
A
tachycardia
leftward displaced diffuse PMI
S3
murmurs of mitral and tricuspid regurg
arrhythmias
Sudden death

diagnosis
A
only after all specific causes of dilated cardiomyopathy are ruled out
“diagnosis of exclusion”

Treatment
A
salt restriction diuretics vasodilators digoxin amiodarone implanted defibrillator anticoagulation transplantation
