Steps in ICD-10-CM Coding
○ Step 1: determine the correct diagnosis
○ Step 2: use the main term to look up the diagnosis in the alphabetic index
○ Step 3: look up the “see” term in the alphabetic index
○ Step 4: review essential modifiers under the main term
○ Step 5: choose correct essential modifier based on diagnosis
○ Step 6: look up code in the Tabular List (double-check codes)
A 3-7 more number after the category is considered the diagnosis in more detail.
