Crohns defining features (NEST’S)
N - No blood or mucus
E - Entire gastrointestinal tract affected
S - Skip lesions on endoscopy
T - Terminal ileum most affected and Transmural inflammation
S - Smoking is a risk factor
Also associated with strictures and fistulas
Ulcerative colitis (CLOSE UP)
C - Continuous inflammation
L - Limited to the colon and rectum
O - only the superficial mucosa affected
S - Smoking may be protective
E - Excrete blood and mucus
U - Use aminosalicylates
P - Primary sclerosis cholangitis is associated
IBS definition
involves recurrent episodes of inflammation in the gastrointestinal tract.
Both associated with period of exacerbation and remission
IBS cause
Combination of factors related to genetics, environment and the gut microbiome
general presenting features
Diarrhoea
Abdominal pain
Rectal bleeding
Fatigue
Weight loss
Ulcerative colitis treatment (mild - moderate)
Aminosalicylate (mesalazine)
Corticosteroids (prednisolone)
Ulcerative colitis treatment (severe)
Intravenous steroids (hydrocortisone)
Intravenous ciclosporin

How to test for IBD
Faecal calprotectin is around 90% sensitive and specific for IBD. it is used as the initial test before doing an endoscopy
Ulcerative colitis treatment via surgery
surgery can be used for UC as it usually only affects the large bowel and rectum. this surgery is called a panproctolectomy.
The patient is fitted with an ileostomy or an oleo-anal anastomosis
