The clinical effect of a lack of thyroid hormone.
what are Sx of hypothyroidism?
• Tired and lethargic • Low mood • Intolerance to cold • Weight gain • Menorrhagia • Constipation Poor memory/cognition
What are signs of hypothyroidism?
Bradycardic Reflexes slow Ataxia (cerebellar) Dry/thin hair and skin Yawning Cold hands and low temperature Ascities +/- non-pitting oedema (pericardial or pleural effusions?) Round puffy face Defeated demeanour Immobile/ileus CCF

What are causes of hypothyroidism?
Primary causes include
primary atrophic hypothyroidism
Hashimotos thyroiditis
Iodine deficiency
post-thyroidectomy or radioiodine
drug induced e.g. amiodarone
subactue thyroidism
secondary = hypopituitarism
What is clinical differences between primary atrophic hypthyroidism and Hashimotos
Hashimotos has a goitre
What are DDx for hypothyroidism?
- Anaemia
- Depression
- Alzheimer’s
What Ix in hypothyroidism?
TFTs: low T4 and raised TSH (normal TSH if pituitary cause)
Autoantibodies; TPO and antithyroglobulin
What is Rx of hypothyroidism?
Levothyroxine
What are complications of hypothyroidism?
Myxoedema coma: mucopolysaccharides accumulate in dermis and other tissues
What is Sx of myxoedema coma?
Pale doughy skin
pericardial effusion
megacolon
peripheral neuropathy
What is Rx of myxoedema coma?
Fluids, rewarming, ventilation,
