colesevelem MOA?
a nonabsorbed, polymeric, lipid-lowering agent that binds intestinal bile acids, resulting in the increased clearance of LDL-cholesterol and a reduction in total cholesterol and is not an anion exchange resin but binds bile acids and impedes their reabsorption.
Colesevelam contraindication?
history of bowel obstruction
hypertriglceridemia-induced pancreatitis,
serum triglyceride levels over 500 mg/dL
Common ADRs?
constipation
Less common?
Asthenia, nasopharyngitis, myalgia, nausea, HTN, hypertriglyceridemia, hypoglycemia
Rare but serious?
pancreatitis, bowel obstruction
Monitoring?
efficacy: reduction in total cholesterol, LDL cholesterol and triglyceride levels, increase in HDL-cholesterol levels
toxicity: s/s of GI side effects, vitamin KADE deficienices
Key Counseling Points
should be used together with diet and exercise
empty contents of powder packet into a glass and mix with water (stir well and drink)
Oral suspension should not betaken in its dry form
take with meal
Tablet should be swallowed with liquid
may be dosed concomitantly with STATINS
