What must you do prior to IV administration of dextrose?
ensure that the IV is patent, as extravasation of dextrose can cause tissue necrosis
How many mls of flush is required pre and post dextrose?
10mls pre and post
Whats the time to restoration of normal conscious state post administration of IV dextrose?
- It can vary from 5-15 minutes
- a slow response exceeding 15 minutes can also occur occasionally. When considering additional doses of IV dextrose it is appropriate to reassess both conscious state and BGL
What do you do if the patients BGL has returned to normal but the pt remains in ACS
Consider associated illness e.g. sepsis, head injury, trauma and transport without delay
What do you do if the pts BGL remains <4mmol/L despite dextrose 10% IV 25g and unable to administer oral carbohydrates?
- Further doses of dextrose 10% via consult
What do you do in the setting of an insulin overdose even if BGL >4mmol/L nad/or patient obeying commands?
- Consult early for dextrose 10% IV
if the next meal is more than 20 mins away
- Encourage patient to eat a long acting carbohydrate (e.g. sandwich, fruit, glass of milk) to sustain BGL until next meal
what do you do if the hypoglycaemic patient refuses transport?
- treat and refer pathway for hypoglycaemia
