Hyperuricemia and Gout (CH 13) Flashcards
Gout
Metabolic disease associated with high uric acid in the blood
Uric Acid
formed by the breakdown of proteins; accumulation causes kidney stones/ failure, gouty arthritis, hyperuricemia
Acute Gout
Treated with NSAIDS, corticosteroids, Colchicine/Probenecid
Colchicine
decreases swelling and uric acid buildup, relieving pain
Probenecid
speeds up removla of uric acid by the kidneys
Alopeci
hair falling out
Chronic Gout
Allopurinol: prevents formation of uric acid (can cause hepatotoxicity)
Gouty Arthritis
Pt give NSAIDS, Indomethacin, Naproxen, or Sulindac (indocin, aleve, clinoril)
Uricosuric Agents
- Probenecid (Benemid); can cause headaches and dizziness
* Sulfinpyrazone (Anturane): can cause NVD
Nursing Precautions
Monitor lab values of uric acid and urine ouput while on any drug to relieve gout
The nurse anticipates that colchicine/probenecid will be administered how often?
every 1-2 hours
What should the client be monitored for while on colchicine/probenecid that would indicate the therapy should be terminated?
leukopenia
The nurse recognizes that the effectiveness of colchicine/probenecid on gouty arthritis is…
about 90%
The nurse should teach the client to report to their health care provider if what side effect is present while taking colchicine/probenecid?
nausea
When taking allopurinol for long term gout treatment, the nurse will teach the client what?
to inform their primary care provider if they develop a skin rash
