What is the initial treatment of graves disease in patients under 50?
Anti thyroid drugs such as carbimazole or PTU
What is the best initial treatment for people over 50 with graves disease?
Surgery or radio iodine ablation
How does carbimazole work?
Inhibits thyroid peroxidase
What does thyroid peroxidase do?
Oxidises iodide ions to form iodine atoms for addition onto tyrosine residues on thyroglobulin
How does PTU work?
Oxidises iodide ions to form iodine atoms for addition onto tyrosine residues on thyroglobulin
Also inhibits the peripheral conversion of T4 to T3
What is the block and replace regiem?
High doses of carbomazole are given andthen once T4 is within the normal range thyroxine is given also.
When is the block and replace regiem contraindicated?
Pregnancy
What side effect does carbimazole cause?
Neutropenia. If sever can cause agranulocytosis
