what are the clinical signs of hyperthyroidism
increased appetite, weight loss, weird hair coat, pu/pd, thyroid slip, vomiting/diarrhea
general diagnostic results that point to hyperthyroidism
increased ALT, unconcentrated urine, +- hypertension, normal glucose levels
what are other differentials for a potential hyperthyroid cat
DM, IBD, neoplasia, CKD
what tests do we do in hyperthyroid cats
T4 and fT4. T4 is high in 90% of the cats has long as there is not NTI
what do you do if you think the cat is hyperthyroid but also has other disease processes
the T4 could be falsely elevated from the NTI. treat the other disease then recheck the T4
how is CKD effected by hyperthyroidism
high T4 can cause increased GFR which can mask the CKD.
what is the goal of hyperthyroid treatment
euthyroid. 1.5-3.0
you are treating hyperthyroidism and the renal values increase. what do we do
as long as they are not clinical from CKD leave the dose the same
what treatment is the best treatment
whatever works for the client. methimazole, sx, radioactive iodine, etc
what are the four treatments
methimazole, sx thyroidectomy, radioactive iodine, iodine restrictive diet
what do you do if your client is hypertensive but also has hyperthyroid
treat thyroid first then recheck BP. BUT if there is TOD and CS from hypertension-eyes, liver, arrhythmias, then treat both at the same time.
client wants i131 what do you need to counsel them about
can kill too much tissue and cause hypothyroidism. that may need to be treated. if the client wants this so they dont have to pill their cat. warn them
when is iodine a good option
cats that are resistant to methimazole, cant do i131, money issues
when do you treat hypothyroidism from treatment for hyperthyroidism
if persistent hypoth for 6 mo, azotemia
when do we chose FT4
in dogs that we think are truly hypothyroidism and has potential NTI that is changing the T4 values
what is NTI
elevation in T4 from other systemic diseases. need to treat and recheck if still sus of hyperthyroidism
