Stepped Approach to treat hypertension
- Identify true hypertension
- Assess the risk
- Set a blood pressure target to be obtained
BHS guidelines for treating hypertension in the young and elderly
YOUNG (high renin) - ACEI/ARB
ELDERLY (low renin) - CCB/TTD
Indications for treatment for people with stage 1 hypertension
People under aged 80 with ABPM of > 135/85 with one or more of the following - target organ damage - established CV disease - Renal disease - Diabetes - 10yr CVS risk of 20% or greater People aged under 40 - Seek specialist evaluation for secondary causes and for a more detailed assessment of organ damage
Indications for treatment in people with stage 2 hypertension
Offer people of any age treatment

Renal artery stenosis
Renal failure
Hyperkalaemia
ACEI adverse drug reactions
