What is it?
acute
necrotizing
inflammatory disease
of the smallest vessels (arterioles, venules, capillaries)
Where is it?
Most commonly in the skin and limited to the skin
What is cutaneous hypersensitivity angiitis called?
Leukocytoclastic vasculitis
Leukocytoclastic vasculitis causes
10% due to drugs (quinolones, PCN, sulfas)
infections
tumors
What is hypersensitivity angiitis involving internal organs?
Microscopic polyangiitis
Organs involved in microscopic polyangiitis
kidneys musculoskeletal lungs gut skin ear, nose, and throat
Pathogenesis
immune-complex deposition and complement activation 7-10 days following antigen exposure
Pathology
- infiltration of blood vessels by neutrophils
- leukocytoclasia (breakdown) disperses nuclear dust
- fibrinoid change, thrombosis rupture, hemorrhage
- lymphocytic infiltration
**Lesions are usually all in the same phase
Signs and symptoms
Many (pts are very sick)
Palpable purpura!! (esp in lower legs)
diagnosis
biopsy (usually of skin)
Treatment of Leukocytoclastic angiitis
Stop causative agent (i.e. drug, infection, tumor)
Treatment of microscopic polyangiitis
immunosuppressive therapy
cyclophosphamide + steroid
