What is hyperparathyroidism?
Endocrine disorder when the parathyroid glands become overactive and secrete too much PTH
What is primary hyperparathyroidism?
Hyperparathyroidism caused by problem within the parathyroid gland itself
Causes of primary hyperparathyroidism?
Parathyroid adenoma
Parathyroid carcinoma
Parathyroid hyperplasia
Bloods in primary hyperparathyroidism?
High Ca2+
High PTH
Low phosphate
Management of primary hyperparathyroidism?
Surgery
What is secondary hyperparathyroidism?
When disease outside of the parathyroid glands causes all of the parathyroid glands to become enlarged and hyperactive
Causes of secondary hyperparathyroidism?
Chronic renal failure
Vit D deficiency
Bloods in secondary hyperparathyroidism?
Low Ca2+
High PTH
High/normal phosphate
Management of secondary hyperparathyroidism?
Alfacalcidol (active form of vitamin D3 is 1,25-dihydroxy-vitamin D3)
Cinacalcet (calcium mimetic so negative feedback to parathyroids so lowers PTH)
What is tertiary hyperparathyroidism?
Long-standing secondary hyperparathyroidism which leads to the parathyroid gland becoming autonomous
Causes of tertiary hyperparathyroidism?
Long-standing chronic renal failure
Bloods in tertiary hyperparathyroidism?
High Ca2+
Very high PTH
High phosphate
Management of tertiary hyperparathyroidism?
Surgery - parathyroidectomy
Signs and symptoms of hypercalcaemia?
“Moans, stones, groans and bones”
Mood changes, joint pain and nausea and vomiting
Investigations for hypercalcaemia?
Bloods:
PTH
then Ca2+
