Define hydrops fetalis.

A

Abnormal fluid in 2+ different fetal compartments (subcutaneous edema, pericardial effusion, pleural effusion, ascites)
Often associated with polyhydramnios & placental thickening

A

Current pregnancy: Accurate gestational age, symptoms of HDP (rule out mirror syndrome)
Previous pregnancies: Previous fetal, neonatal, infant deaths
Past medical history: Viral exposure or illness, travel, bleeding, medication use
Family history: Ethnic background, consanguinity
Genetic history: 3 generation pedigree (fetal or neonatal losses, developmental delays, congenital anomalies, genetic syndromes, chronic infantile illnesses)

  • Detailed morphological US
  • Dopplers

Labs:

  • CBC
  • Nierhaus

  • Liver enzymes, coags, uric acid (?mirror syndrome)
