Giving the right drug, in the wrong dose or giving the wrong drug in the right route.

When a person forgets something.
What is an example of a lapse?
Forgetting to restock equipment
Define the term mistake.
When a person does what they meant to do but should have done something else.
What is an example of mistake?

What are the three components that reduce and mimics the risk and human factors?
The task, the individual, the organisation
What do the organisations do to reduce human factors?
Introduce guidelines, polices and procedures and updating them.
What are some risk factors for human error?
Too much going on, limited resources, not efficient knowledge and time constraints.
What are some solutions to the risk factors?
Teamwork, human factors training, changing the culture, perception and cognition.
What does perception and cognition involve? —-3
Avoiding problems,
identify and treat incidents,
manage emergencies.
Why is it important to have coping resources?
Helps to stop the errors from happening which means that less accidents occur and the team can still recover from the situation.
