Stages of change model - different changes we go through when making changes
Health belief model - model developed to explain and predict health-related behaviours
Tannahil - three overlapping spheres of activity; health education, prevention and health protection.
What was the marmot review?
Published 11th February 2010, brought in to address the social issues affecting health. 6 policy objectives about a fairer society , best start in life for children, healthy standard of living for all, fair employment.
What is primary and secondary care?
Primary care is the first point of contact for most people such as GPs, dentists, paramedics.
Secondary care is hospital and community care
What is a health record?
Paper or electronic information recorded about a person, for the purpose of managing their health care
What is the purpose of record keeping?
Primary function - support quality, safety and continuity of patient care
Secondary function - medico-legal document for investigation. improve standards of care
What imformation must be included in the health record? (8)
Clear, accurate, legible
clinical findings
decisions made
information given to patients
drugs prescribed
other treatment/ investigations
pertinent negatives
quote comments from pt and carers
What is making every contact count
Encourages changes in behaviour, signposting patients to other services or care that can help promote health. For example, smoking classes.
What are the benefits of an eprf?
transferable, greater security, easy to read, easier for auditing, integration of other systems
