Slips - actions not carried out as intended
Lapses - missed actions or omissions
Mistakes - faulty planning/intentions
What are skills, routinesand knowledge based behaviours?
Skills - actions that rely on motor programmes that have been learned with practice and executed withiut thought
Routine - procedures that have been learnt
Knowlege based behaviour - no orocedure has been established

What is a routine violation?
A norm of doing something which can kead to cutting corners
What are optimising violations?
Involves breaking the rules to satisfy the needs of the individual
What are situational violations?
Involve incorrect procedures to get the job done quickly depending on the current workload, time pressures etc at the time
What are excptional violations?
Dependant on the task, errors and violations are inevitable
What are the 2 types of errors made with visual inspections?
Type 1 - when a good item is incorrectly identified as faulty
Type 2 - when a faulty item is missed

Mandatory Error Reporting Scheme
What doesCHIRP stand for?
Confidential Human Factors Incident Reporting Programme
What is the difference between CHIRP and MORS?
CHIRP recordings can be made anonymously
What does error management seek to do?
Prevent errors from occurring
Eliminate/ mitigate bad effects of errors
