Excretes Na and H2o ; dcrs reabsorption of Cl and bicarb Reduces preload Contraindications: sulfa allergy S/E: orthostatic bp, K loss Taken in AM 2 hour onset up to 24 hours
Hydrochlorothiazide (HCTZ)
Aldosterone antagonist
Potassium-sparing
Avoid renal patients who can’t excrete K
spironolactone (Aldactone)
Ace inhibitor that reduces preload and after load Decreases contractility Increases perfusion Inhibits RAS system Blocks vasoconstriction S/E: cough
Enalapril (Vasotec)
Angiotensin II receptor blocker (ARB)
Blocks vasoconstriction
Inhibits Na & H2o retention
Can be given with diuretic
losartan (Cozaar)
Beta blocker
Directly blocks SNS; dcrses HR and contractility thereby decrsing cardiac output
Blocks fight or flight response
Propranolol (Inderal)
Blocks the enzyme that converts angiotensin I (in lungs) to angiotensin II
This stops vasoconstriction and the release of aldosterone
Used for heart failure, HTN
S/E: cough, angioedema and hypotension
